1. Who We Are
ForHealthInsurance.com is operated by Vista Health Solutions, Inc. (“Vista Health,” “we,” “us,” or “our”), a licensed health insurance brokerage that helps individuals, families, the self-employed, and small businesses compare and enroll in health insurance plans. This Policy applies to the website www.forhealthinsurance.com and the services we provide through it.
2. Disclaimers & Non-Affiliation
Vista Health Solutions, Inc. is an independent insurance brokerage. We are not affiliated with, endorsed by, or connected to any government agency, and we are not affiliated with any State or Federal Health Insurance Marketplace (such as HealthCare.gov).
Any premium, subsidy, or tax-credit figures shown on this website are estimates for informational purposes only; actual premiums, subsidies, and advance premium tax credits can only be determined and offered by the applicable insurance carrier or Health Insurance Marketplace. The only way to apply for premium tax credits is through the applicable Health Insurance Marketplace.
Information on this website is provided for general educational purposes only and does not constitute medical, legal, tax, or financial advice.
3. Compensation & Broker Fees
When you enroll in a plan through us, we typically receive commission compensation directly from the insurance carrier. This commission is already included in the plan’s premium and is not an additional charge to you — the price you pay for a given plan is the same whether you enroll through us or directly with the carrier. We do not charge you a fee to shop for, compare, or enroll in coverage, and no fees are ever added to Marketplace plans.
4. Information We Collect
We collect only the information needed to prepare your quotes and process the applications you ask us to submit. This may include:
- Identifiers & contact information — name, mailing address, email, and telephone number, for you and (where applicable) your spouse and dependents.
- Demographic & eligibility information — date of birth, ZIP code, household size, income estimates, and citizenship/eligibility information used for plan and subsidy estimates.
- Information required by the insurer — where an insurance carrier requires it to process your application, this may include a Social Security number, Medicare identifiers, tobacco use, or health-related information. We collect this only as needed to submit the application you request.
- Business information — optional details such as business type and start date.
- Technical & usage data — IP address, device/browser information, approximate location, and pages visited, collected via cookies and analytics tools (see §12).
- Communications — records of your calls, chats, texts, and emails with us. Please note that our telephone calls are recorded for quality, training, and recordkeeping purposes.
5. Notice at Collection
When you use our online quote and enrollment form, we collect the categories of information described in Section 4 for a single purpose: to prepare your quotes and to process the insurance application you ask us to submit. We do not use the information you provide on that form to market other products or services to you, and we do not provide it to third-party marketers. If you consent to be contacted (see Section 8), we use your contact details only to help you with the coverage you have requested. Links to this Privacy Policy and to your privacy choices are provided at the point of collection.
6. How We Use Your Information
- To prepare quotes and process the insurance applications you request;
- To contact you about the plans, quotes, and coverage you have expressed interest in;
- To provide customer service throughout application, enrollment, and renewal;
- To operate, secure, and improve our website and services;
- To comply with legal, regulatory, insurance-licensing, and carrier-contract obligations.
We do not use your information for third-party marketing or advertising.
7. How We Share Your Information
We do not sell your personal information, and we do not share it with third-party marketers, advertisers, data brokers, or outside service providers or vendors for their own purposes.
We share your information only in these limited circumstances:
- Insurance carriers and their agents — to obtain the quotes and submit the applications you request. This is the core of what we do as your broker.
- Our own licensed insurance agents — who assist you directly with your quote, application, or coverage.
- Legal & regulatory recipients — when required by law, subpoena, or to protect rights and safety.
- Business transfers — in the event of a merger, acquisition, or sale of assets, subject to this Policy.
If you later choose to provide information directly to an insurance carrier or another third party, that information is governed by their privacy policy, not ours.
8. Calls, Texts & Email Consent (TCPA)
By submitting your contact information through our website, you consent to be contacted by Vista Health Solutions, Inc. and its licensed agents by telephone, text message, and/or email regarding health insurance quotes, applications, and related services you have requested — including, where applicable, through an automatic telephone dialing system or a prerecorded or artificial voice — at the telephone number(s) you provide, in accordance with the Telephone Consumer Protection Act (TCPA). Consent is not a condition of purchase. Message and data rates may apply. Our calls are recorded for quality and recordkeeping purposes.
You may revoke consent or opt out at any time by replying STOP to a text message, telling a caller that you wish to opt out, or contacting us using the information in Section 17. You may still receive non-promotional communications about a quote, application, or policy you have requested.
9. HIPAA, GLBA & Insurance Privacy
HIPAA. The Health Insurance Portability and Accountability Act (HIPAA) governs Protected Health Information held by covered entities such as insurance carriers and health providers. We do not collect HIPAA-protected health information through our website, and we do not request it from applicants. If you ask us to contact a health insurance company on your behalf in a situation that involves protected health information, we will inform you of the applicable HIPAA authorization procedure before doing so.
GLBA & State Insurance Privacy. Nonpublic personal financial information you provide in connection with seeking insurance is handled in accordance with the Gramm-Leach-Bliley Act and applicable state insurance privacy laws. Because we do not share your information with nonaffiliated third parties for their own use (see Section 7), our information practices are designed to keep your data within the process of obtaining the coverage you request.
10. Consumer Health Data
Some state laws (for example, Washington’s My Health My Data Act) regulate “consumer health data” separately from HIPAA. Because we do not sell personal information, do not share it with third-party marketers or advertisers, and do not use it for targeted advertising, the practices those laws are primarily designed to address do not apply to how we handle your information. We use any health-related information solely to prepare and submit the insurance application you request, and we do not repurpose it for marketing or share it outside the limited circumstances described in Section 7.
11. Your State Privacy Rights
Depending on your state of residence, you may have rights regarding your personal information, which can include the right to:
- Know / Access — request the categories and specific pieces of personal information we have collected, the sources, the purposes, and the categories of any third parties with whom we have shared it;
- Delete — request deletion of personal information we collected from you, subject to legal and recordkeeping exceptions (see Section 13);
- Correct — request correction of inaccurate personal information;
- Portability — request a copy of your information in a portable format, where applicable;
- Opt out — of any sale or sharing of personal information. Note: we do not sell or share your personal information (see Section 7), so there is nothing to opt out of in that respect;
- Non-discrimination — we will not discriminate against you for exercising any of these rights.
To exercise any of these rights, contact us using the information in Section 14. We will verify your request and respond within the timeframes required by applicable law. You may use an authorized agent to submit a request where the law permits, and you do not need to create an account with us to make a request.
12. Cookies & Analytics
We use cookies and similar technologies to operate the website, remember your preferences, and understand how our site is used. For website analytics, we use Google Analytics and Google tags. These tools collect technical information such as IP address, device and browser details, and the pages you view, to help us measure and improve our website’s performance. We do not use advertising pixels, session-replay tools, or third-party advertising networks.
You can control cookies through your browser settings; disabling some cookies may affect how the website functions. To learn how Google Analytics collects and processes data, you can review Google’s information at google.com/policies/privacy/partners.
13. Security & Retention
We maintain administrative, technical, and physical safeguards designed to protect your personal information. No method of transmission or storage over the internet is completely secure, and we cannot guarantee absolute security.
Retention. We retain personal information for as long as needed to provide our services and to meet our legal, regulatory, and insurance-recordkeeping obligations. As a licensed insurance brokerage, we follow applicable state insurance-recordkeeping laws and guidance from the National Association of Insurance Commissioners (NAIC), which generally call for retaining transaction and customer records for a number of years after the last transaction. Consistent with this guidance, we retain telephone call recordings and related records for up to ten (10) years. Retention periods may vary by record type and by the requirements of the states in which we operate.
14. Privacy Requests
To submit a request to access, correct, delete, or obtain a copy of your information, contact us at [email protected] or using the details in Section 17. We do not require you to create an account to submit a request. We will verify your identity, honor authorized-agent requests where required by law, and respond within the timeframes the law provides. Some information may be subject to retention requirements (see Section 13) and cannot be deleted until those obligations expire.
15. Children & Dependents
This website is intended for adults. We do not knowingly collect personal information directly from children under 13. Where a parent or guardian provides a dependent’s information for insurance purposes, that information is handled as part of the adult’s request for coverage.
16. Changes to This Policy
We may update this Policy to reflect changes in our practices or the law. We will revise the “Last Updated” date above and, where required by law, provide additional notice and obtain your consent before applying material changes to information previously collected. We review this Policy at least annually.
